The Rayant Dental Option Plan offers a great way to save premium dollars without reducing benefits.
Members have the freedom to receive dental services from any dentist. Members that choose a
dentist participating in the Rayant Dental Option Plan can:
- Maximize benefits while reducing out-of-pocket costs
- Receive discounts off participating dentists' charges, which range between 10 and 30%
- Have the option of selecting from over 10,000 participating dentists located in New York,
New Jersey and Pennsylvania
With any choice, members are always covered for eligible services because of the plan's out-of-network option.
The Rayant Dental Option Plus Plan
This plan offers an even richer level of benefits than the Rayant Dental Option Plan.
It provides incentives for members to choose the more deeply discounted Rayant Dental PPO network.
Additionally, variable coinsurance options result in even greater savings and lower out-of-pocket costs.
The Rayant Dental PPO Plan
In Network
The Rayant Dental PPO Plan, a Preferred Provider Organization Plan, offers the lowest fees available to
members through our Rayant Dental PPO Network of participating dentists.
Out of Network
Members may go to a nonparticipating dentist and may have to pay the dentist his/her usual fees at the time of service.
Members must then file claims for reimbursement, which is based on our reduced allowances, and are responsible for any charges in excess of these amounts.
The Rayant Dental PPO Access Plan
This plan is an ideal way to stretch benefit dollars while providing members with valuable
benefits and savings. Our Rayant Dental PPO Access Plan uses the same network of participating
PPO dentists as our Rayant Dental PPO Plan, but at our most affordable premiums.
This program provides:
- Eligible preventive and diagnostic services
- Simple amalgam fillings at no cost to the member
- Added advantages for major or specialty services, such as root canals, crowns
and bridges, by paying only the reduced fee directly to the dentist
Members receive benefits only when visiting a participating Rayant Dental PPO Network Dentist.
The Rayant Dental EPO Plan
This plan offers members the most for their premium dollars with no hassles, no commitment to any one
dentist and richer benefits, including:
- 100% coverage for most eligible preventive, diagnostic and restorative services
- Low out-of-pocket costs for eligible major services
- A large network of dentists and specialists
Members do not need to select a primary care dentist or submit referrals or claim forms.
