The Rayant Dental Option Plan is a great way to save premium dollars without reducing benefits.
Members have the freedom to choose from several options each time a covered service is needed.
Choice of dentist will affect the plan reimbursement and the member's out-of-pocket costs.
A Good Deal
Members may visit any dentist they wish, even if the dentist does not participate in one
of the Rayant dental provider networks. These dentists generally charge their usual fees
and may require full payment at the time of service. In addition, members must file claims for
reimbursement, which is based on our reduced allowances. Members are responsible for
any charges in excess of these amounts.
A Better Deal
Members may also visit any dentist participating in our Rayant Dental Traditional Network.
Members realize out-of-pocket savings because these dentists accept our reduced allowances
as payment in full, less any applicable deductible and/or coinsurance.
The Best Deal
By selecting a dentist identified by either an asterisk from the Rayant Dental Traditional
Network print directory or bold-face type in the Rayant Dental Option Plan Network Directory
on our Web site, members realize the greatest out-of-pocket savings. These dentists accept
our most deeply reduced allowances as payment in full, less any applicable deductible and/or
coinsurance.
The Rayant Dental Option Plus Plan
The Best Deal Made Even Better
This plan offers members an even richer level of benefits than the original Rayant Dental Option Plan.
Variable coinsurance and deductible options allow members to realize greater savings and lower
out-of-pocket costs.
Choice of dentists affects plan reimbursement and the member's out-of-pocket costs. The Rayant
Dental Option Plus Plan offers the same network flexibility as the Rayant Dental Option Plan.
The Rayant Dental PPO Plan
In Network
This plan, a Preferred Provider Organization Plan, offers the lowest fees available to our
customers through our Rayant Dental PPO Network of participating dentists. These dentists
accept our reduced allowances as payment in full, less any applicable deductible and/or
coinsurance.
Out of Network
Members may go to a nonparticipating dentist and may have to pay the dentist his/her usual
fees at the time of service. Members must then file claims for reimbursement, which is based on our
reduced allowances, and are responsible for any charges in excess of these amounts.
The Rayant Dental PPO Access Plan
This plan is an ideal way to stretch benefit dollars while providing members with
valuable benefits and savings. Our Rayant Dental PPO Access Plan uses the same network
of participating PPO dentists as our Rayant Dental PPO Plan, but at our most affordable
premiums.
This program provides:
- Eligible preventive and diagnostic services
- Simple amalgam fillings at no cost to the member
- Added advantages for major or specialty services, such as root canals, crowns
and bridges, by paying only the reduced fee directly to the dentist
Members receive benefits only when visiting a participating Rayant Dental
PPO Network Dentist.
