Group Life Insurance:

• Guarantee Issue Life and Accidental Death and Dismemberment Insurance (Group Life/AD&D)
• No medical questions or exams for life coverage up to $35,000 on each employee for groups with 2-5 employees, $50,000 for groups with 5-9 employees, and $100,000 or more for groups with 10 or more employees
• Our preferred carrier for Group Life/AD&D is Fort Dearborn Life ('A+' Superior, AM Best rating)

Group Short-Term Disability (STD):

• Group STD plans available for groups with 10 or more employees
• Group STD will supplement New Jersey Temporary Disability Benefits

Group Long-Term Disability (LTD):

• Group LTD plans for groups with 2-9 employees are available through Fort Dearborn Life (FDL) when written with a minimum of $15,000 in Life Insurance
• Group LTD replaces 60% or more of income to age 65
• FDL standalone LTD plans are available for groups of 10+ employees

Voluntary Life and Long-Term Disability Insurance:

• Voluntary Term Life Insurance with no participation requirements for a group's employees, spouses, and/or dependents is available
• Guarantee issue amounts are also available when minimum participation requirements are achieved
• Plans include portability option, waiver of premium benefit, and living benefits for the terminally ill at no extra cost
