Dental Consultant Corner
To review your paper claims and predeterminations as quickly as possible, please submit the following documentation for the listed services:
- Onlays, Veneers & Crowns = Recent PA that displays the tooth apex
- RCT Retreatment = Pre-retreatment radiograph, reason for retreatment, and date of original RCT
- Scaling/Root Planing, Osseous Surgery, Gingivectomy = Recent periodontal charting and FMX
- Soft Tissue Grafting = Recent periodontal charting and narrative indicating amount of remaining attached gingiva
- Crown Lengthening = Recent PA
- Implants = Recent FMX
- Fixed and Removable Prosthesis = Recent FMX, if a replacement, age of previous prostheses and
reason for replacement
- Surgical Extractions = Recent PA
Always send readable X-rays. If the image isn't clear to you, it will be difficult for the consultant to read.
And, if necessity is not evident on the X-ray, please include a narrative.
Never send in your original X-rays. Submit duplicates or use double film packets.
Root Canal Treatment — X-rays and Predeterminations
Rayant has adopted the following policy with regard to requiring X-rays for payment for root canal therapy:

If you receive a predetermination for RCT that requests a post-operative radiograph, and the patient is in the MAC or PPO
plans, you may ignore that request upon completion of treatment when you file for payment. Merely insert the date of service
on the predetermination form and return it.
